Writing your report
Please be as specific as possible when describing the violation. Include locations, times, dates, and who was involved, including people and departments. Don't worry about writing style or format.
Your contact information and confidentiality
Your contact information is not required to report a violation. However, there may be cases where we are unable to fully investigate an issue without communicating with you.
If you choose to provide your contact information, it will only be used to follow up with you on your report, if needed.
Your name will not be disclosed under Freedom of Information requests.
